Overview

Course Title and Description:

HLT POL X 407.8-Care Planning and Patient Navigation

 

Care planning and navigation are key skills that all patient advocates will use in their daily work. This culminating course outlines the critical knowledge and skills necessary for advocates to create roadmaps for patient navigation and care planning. Course work provides the opportunity for students to integrate the knowledge and practice some of the skills they have acquired in the prerequisite courses utilizing simulated patient case studies. Topics include importance of documentation; scope of practice; practice specialization; motivational interviewing; the art of conducting assessments; community resources; working through difficult patient situations; and formulating a self-care plan. Students create a simulated patient case study in the first week of class and develop a care plan and roadmap for navigation which is presented the last week of class. In addition, they participate in a patient management meeting every three weeks with their assigned team members to discuss patient cases and resources for care planning.

Specific Instructor Duties

  • Update course syllabus in consultation with the UCLA Extension Program Director using UCLA Department of Community Health Sciences approved syllabus
  • Prepare and deliver course materials and learning assessments in assigned format
  • As clearly outlined in the syllabus, communicate the Teaching Objectives and Specific Learning Outcomes to students and review the grading policies of the course
  • Throughout the quarter, provide timely feedback on outcome of all learning assessments. Post individual student scores on secure Canvas LMS no later than one week from the date of exam/assignment
  • Evaluate student achievement of Specific Learning Outcomes and assign grades
  • Post final student grades to the transcript system in a timely manner no later than 2 weeks after the last date of course meeting
  • Respond to student questions and learning needs in a timely manner
  • Communicate about administrative matters with Program Director and Program Representative in a timely manner
  • Utilize the Canvas LMS as the support platform for this course
  • Stay current regarding the professional body of knowledge in the field
  • Complete required administrative tasks in a timely manner including: complete all hiring paperwork; submit updated quarterly syllabus; post bio and photo on the UCLA Extension website; review and sign quarterly contract; order required texts; communicate AV and classroom set-up needs
  • Employ cultural humility and competent teaching methodologies in the classroom inclusive of a diverse domestic and international student population
  • Respond to student inquiries about final grades and consult with Program Director as needed
  • Keep final exams/papers for up to 13 weeks following the last class session of each course and allow students to access them during that time period

 

Minimum Qualifications

  • Minimum of a Master’s in Social Work, Nursing, or Public Health
  • College-level teaching experience preferred
  • Must be committed to maintaining the highest level of academic standards and integrity
  • Highly effective oral and written communication skills
  • Highly effective interpersonal skills
  • Proficiency in (or willingness to learn) the use of instructional technology and online learning management systems including the Canvas Learning Management System
  • Active participation in continuing education and professional development

 

Special Conditions of Employment

 

Instructors are hired on a quarterly contract basis.

Because Extension is a division within UCLA, all Extension Program instructors and courses must be formally approved according to the regulations of the Academic Senate of the University of California. Eligibility to teach a course is contingent upon this formal academic approval. Once approved, teaching assignments are “by agreement.” The Instructor’s Contract outlines the deliverables for the course, the course schedule, and the compensation terms, subject to Extension policies and procedures. UCLA Extension makes no commitment to hire an instructor until a signed Instructor Contract has been completed. Should the course section an instructor plans to teach be canceled for any reason, the Instructor Contract, including rights to compensation for future section meetings, is voided.
